Complexities and simplifications in a heart device clinic

Normative interferences between actor-network theory and evidence-based medicine

On a general level evidence-based medicine (EBM) has since its very beginning in 1992 been beset with polarizing criticism as well as conciliatory attempts while professionals have continuously struggled with the practical details of how to increase the use of evidence in clinical practice. Inspired by actor-network theory (ANT) this paper first gives an account of the decision-making based on evidence et cetera in a heart device clinic. It then investigates the possible interferences and normative implications of an ANT account for the agonistic playing field of EBM. In accordance with work by Annemarie Mol and John Law the work of simplification and complexity is focused. Structured orders as well as more chaotic complexities within clinical decision-making ”based” on evidence stand out. These observations carry normative implications supporting and opposing the project of EBM, adding to existing polarization as well as reconciliation attempts in debates about EBM. While teasing out normative implications of the descriptions, a reconciliatory normativity is performed by the shear increase of complexity. It is suggested that this reconciliatory normativity is depriving the scholar of analytic resources. In order to follow actors, simplification and polarization may have to be a real alternatives and not only attitudes to be overcome by finding evermore complexities. Polarization and reconciliation—as well as complexities and reductions of complexities—are clinically useful attitudes when understanding and doing EBM—however, EBM then not as a general project to embrace or reject, but as practices to engage in and improve.
